1. Authorization to Release. I authorize Xstar Education to release the student identified above to the person(s) listed in this submission, subject to Xstar Education's verification and safety procedures.
2. Identity Verification. Xstar Education may require government-issued photo identification or other reasonable verification. Xstar Education may delay or refuse release when identity or authorization cannot reasonably be verified.
3. No Delegation. An authorized pickup person may not authorize another person to pick up the student. Any additional or substitute pickup person must be authorized by the parent/legal guardian through an approved Xstar Education process.
4. Changes and Revocation. I may modify or revoke an authorization by submitting updated instructions through Xstar Education's approved written or electronic process. A change becomes effective after it is received and reasonably processed.
5. Custody and Court Orders. I represent that I have legal authority to provide these instructions. This authorization does not alter custody rights or court orders. I am responsible for promptly providing Xstar Education with current court documentation that restricts access to or release of the student.
6. Safety-Based Delay or Refusal. Xstar Education may temporarily delay or refuse release when staff reasonably identify an authorization, identity, legal, behavioral, impairment, emergency, or other student-safety concern and may contact a parent/guardian, emergency contact, law enforcement, or other appropriate authority when reasonably necessary.
7. Transfer of Care. After the student is properly released and the applicable sign-out procedure is completed, care, supervision, transportation, and safety responsibility transfers to the receiving person, subject to applicable law.
8. Electronic Records and Signatures. I agree that this authorization may be completed, signed, submitted, stored, and maintained electronically and intend my accepted electronic signature to evidence my authorization to the extent permitted by applicable law.
9. Enrollment Agreement. This authorization supplements the student's applicable enrollment agreement and policies. Except as expressly modified here, applicable terms of those agreements remain in effect.
